INSURANCE AND BILLING

When arriving for your appointment, please provide a current copy of your insurance card and driver’s license.

Please note, if your insurance requires a referral, you must have a written referral sent to our office at least 48 hours prior to your scheduled appointment time. You can bring the referral to our office or have your primary care physician fax the referral to our office. The fax number is 502-326-8589.

Cassis Dermatology & Aesthetics Center does accept most insurance plans; however, please check with your insurance company to ensure coverage.
