Schedule Appointment

Appointments are available Monday through Friday from 8 AM to 5 PM.  Scheduling an appointment is an easy process. To schedule an appointment, simply call our office at 502-326-8588 and press option #1 or email frontoffice@cassisderm.com. You will need the following information:

• Patient Name
• Date of Birth
• Patient Phone Number
• Address
• Reason for Visit
• Insurance Plan

Please note, if your insurance requires a referral, you must have an HMO plan you must have a written referral sent to our office at least 48 hours prior to your scheduled appointment time. You can bring the referral to our office or have your primary care physician fax the referral to our office. The fax number is 502-326-8589.
